We sustain and grow collaboration through convening the Colorado Health Extension System (CHES) -- a cooperative of practice transformation organizations, regional health connectors, State agencies, and other partners interested in health and healthcare in Colorado.

Train the Trainer is focused on developing the knowledge, confidence, and skills to support engagement in Alternative Payment Model (APM) 2 and other value based payment opportunities to Practice Facilitators and Regional Accountable Entity (RAE) liaisons across Colorado.
